*Home Visitors provide weekly 90 minute visits to assess children's development and demonstrate educational activities to build early learning skills
*Screens children within 45 days of entry for identification of potential health or development concerns and access to medical care including transportation to appointments, linking with community resources, and partner with parents on family goal setting
*Provide information on topics that may be of interest to parents including: pregnancy health, childhood and general health, nutrition, parenting, and understanding children's behavior
*Play groups (social groups) are held twice a month and include fun activities, snacks, and community speakers; transportation provided to attend event if needed
