*Offers primary health care to children aged 10 through adults: illness and well checkups, chronic disease management, lab work, radiology, pharmacy; full service dental care for adults; offers pap tests and well woman checkups; however family planning and prenatal care services both referred to other providers
*Mental health case management and medication management; medication management occurs via video (telemental health) with a Nurse Practitioner from Apalachee Center; Most patients receiving mental health services are referred by Apalachee Center, and Apalachee clients can request to go there for appointments
*Provides transportation to and from appointments for patients who qualify
*Works with Bond Community Health Center to ensure patients full care
