AmeriCorps Senior Corps

*Senior Corps is a network of national service programs for Americans 55 years and older, made up of three primary programs: Foster grandparents who serve one-on-one as tutors and mentors to young people with exceptional needs, Senior companions to help home bound seniors, and RSVP volunteers who conduct safety patrols, renovate homes, and respond to natural disasters.

AmeriCorps VISTA (Volunteers in Service to America)

*National service program administered through the Corporation for National & Community Service, dedicated to ending poverty by building the capacity of nonprofit organizations and public agencies *Members serve full time for 1 full year at a community organization (nonprofit, college/university, local government office, etc.

*Licensed family and Primary Mental Health Nurse Practitioner (PMHNP) (APRN9315757), board certified by the American Nurses Credentialing Center (ANCC) in practice at Bond Community Health Center who is currently accepting new patients in a location which is accessible and can be reached by public transportation *Fees $120 an hour, offers sliding scale for self-pay; accepts Aetna, Capital Health Plan, CIGNA, FL Blue (BCBS), Magellan, Medicaid, Staywell, and United Health One Insurances *Treats all ages as individuals and families in person, by phone, and by video (telemental health); specialty treatment areas include general anxiety and depression, anger management, psychotic disorders personality disorders, stress management, bipolar disorder, self-esteem, Schizoaffective Disorder, Schizophrenia, substance abuse *Therapist is female, speaks English and Kiswahili, African American & able to prescribe and manage patient's medications *Also practices at Apalachee Center and Ability Plus Mental Health, LLC

Apalachee Center, Children's Outpatient Services

*Community based service for children and adolescents age 6-18 at-risk for psychiatric hospitalization; Children’s Outpatient Program provides psychiatric evaluations, medication management, therapy and case management services; staff provide services in the home, office, or child’s school to accommodate family’s busy schedules *Family Intervention Team (FIT) receives referrals from DCF investigators for families that may be in need of services; the team provides rapid access to mental health and substance abuse services, in an effort to reduce abuse neglect and removal of children from the home *Statewide Inpatient Psychiatric Program (SIPP) assigns a case manager and a therapist work with families who feel that their child’s mental illness has risen to the level that they will need to be placed in an intensive, residential setting; assessment determines whether a placement is needed or if intensive services can be provided to prevent the placement *Community Action Team (CAT) provides full range of services to clients aged 11-21, who failed at less intense forms of treatment or who may have multiple crisis stabilization episodes, involvement with law enforcement or Juvenile Justice systems, involvement with the child welfare system, or current/possible placement in an alternative school setting; services available in Leon, Gadsden and Wakulla Counties *Accepts most insurance, Medicaid, Medicare, sliding scale for those who meet clinical criteria

Apalachee Center, Community Action Team (CAT)

*24/7 Crisis Response Team providing a full range of services to clients aged 11-21 with a documented mental illness, who failed at less intense forms of treatment or who may have multiple crisis stabilization episodes, involvement with law enforcement or Juvenile Justice systems, involvement with the child welfare system, or current/possible placement in an alternative school setting; *A step up from ‘traditional’ outpatient services or a step down from a long-term placement, such as a SIPP; available 24/7 to respond to crisis calls and are deployed when necessary in order to prevent crisis stabilization episodes or law enforcement involvement *Serves a limited number of families at any given time and services last anywhere from six to nine months; team consists of a Team Leader, two Care Managers, two Therapists, 3 Peer Specialists, and one Staff Assistant; Provides limited respite care, not lasting more than four hours for caregivers *Services available in Leon, Gadsden and Wakulla Counties; *Accepts most insurance, Medicaid, Medicare, sliding scale for those who meet clinical criteria

*Comprehensive community-based treatment program in Leon County residents ONLY for persons with severe and persistent mental illnesses, significant functional impairments, and/or history of frequent hospitalizations, substance abuse, and inability to meet basic needs * Uses the P.
