Apalachee Center, Homeless Project

*Provides outreach, mental health case management and psychiatric/medication management and supportive services for homeless persons with severe and persistent mental illness who are uninsured with a goal to provide clinical assessment, supportive services, and stabilize functioning *Outreach at the Kearney Center and street outreach to enroll clients where they are screened and offered intake if needed by therapists and social workers from various agencies involved in Big Bend Homeless Coalition *SSI/SSDI Outreach, access and recovery (SOAR) Services to assist eligible adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder with applications and access.

*Serves persons age 18 and older who have a primary mental health diagnosis and meet clinical criteria providing mental health evaluations, medication management, case management, outpatient individual and/or family mental health therapy, psychosocial rehabilitation services (aka day treatment which includes social rehabilitation and basic living skills training), co-occurring disorder treatment for persons with a mental health diagnosis along with a substance abuse disorder, and primary medical care services *Same Day Access Program for new outpatient patients: walk-in or join a video call during same-day access hours for an intake session to begin services; M, W, F 9AM-11:20AM and Tu and Th from 12:30PM-3PM *Provides Medication Assisted Treatment (MAT) to treat opioid and alcohol use disorders

Apalachee Center, Mobile Response Team (MRT)

*The Mobile Response Team (MRT) operates a 24/7 crisis hotline and provides services for people of any age in a mental health or substance abuse crisis; A feature of their service is in-person response to calls about mental health crisis; the MRT responds to all 8 counties in the Big Bend region but will have limited response depending on staffing resources; If the MRT cannot respond in person, they might offer video conferencing to the caller *The MRT aims to dispatch two counselors if they are available; they only respond to callers with one counselor if the individual is an existing Apalachee Center client that the MRT is familiar with; all other responses will be with two counselors or with law enforcement accompaniment if there is concern for safety in the home environment *After sunset and before sunrise, the MRT does not provide in-person crisis response unless requested by law enforcement or a public safety access point; They only provide in-person crisis response during business hours (8AM-5PM) *The MRT serves to reduce psychiatric hospitalization but will Baker Act or Marchman Act individuals if necessary; If a Baker Act or Marchman Act is necessary, then law enforcement will become involved; the MRT does not respond to calls where a suicide attempt has been initiated or where the crisis person is deemed at risk of harming others; They will recommend or will help contact CDA so EMS or law enforcement can dispatch to the home for the individual that has attempted suicide and needs medical attention

Apalachee Center, NAVIGATE Program

*Comprehensive outpatient treatment program for Leon County residents 18 to 40 years-old assisting persons following their first psychotic episode or in their first year of diagnosis and their families to successfully find their way to psychological well-being, and to access the services they need in the mental health system * Team-based approach including a Program Director, an APRN prescriber, an Individual Resiliency Trainer (IRT), Family Education (FE) Clinician, a Case Manager, a Supported Employment and Education (SEE) Specialist, and a Peer Support Specialist *Provides direct coordination of all medical care, help managing symptoms of illness, immediate crisis response as needed, prescribes and manages medications offers therapy, and as the patient is ready can help obtaining financial entitlements and housing; assistance with housing tasks so a person can live in regular housing alone or with a roommate; learning how to socialize; treatment for clients with co-existing substance abuse (dual diagnosis); employment service and job placement; assistance with legal issues; support, education, and skill-teaching for family members; offsite (in-home) and online (telemental health) meetings available depending on patient's circumstances *Fees based on the needs of the patient; accepts most insurance, Medicare, Medicaid, and offers an income-based sliding scale; fees for medication management may be ongoing

Apalachee Center, PATH Crisis Stabilization Unit

*PATH (Positive Alternatives to Hospitalization) is a short-term, (2-7 days), inpatient crisis stabilization unit for children and adults experiencing an acute mental health crisis providing mental health evaluation and treatment, psychiatric services, physical assessment and medical care as needed, therapy and psychoeducation, and aftercare planning *Accepts most insurance, Medicare, and Medicaid accepted; Services individuals without insurance coverage for inpatient stay, sliding scale fee based on income assessed at admission
