*Central Receiving Facility serving as 24/7 walk-in central receiving facility for anyone experiencing a mental health or substance abuse crisis including suicide crisis intervention and all persons who are subject to the involuntary Baker Act or Marchman Act where evaluation will determine appropriate level of care (inpatient or outpatient) *Persons determined to be in need of outpatient care will be given referrals to receive outpatient services *Based on insurance, preference, availability and other factors, those admitted to inpatient care may go to either Eastside, Tallahassee Memorial Behavioral Health Center, HCA Florida Capital Hospital, or in the case of most uninsured patients, PATH (Positive Alternative to Hospitalization) *Fees depend on need of patient; accepts most insurance, medicare, and medicaid

Apalachee Center, FL Assertive Community Treatment (FACT)

*Comprehensive community-based treatment program in Leon County residents ONLY for persons with severe and persistent mental illnesses, significant functional impairments, and/or history of frequent hospitalizations, substance abuse, and inability to meet basic needs * Uses the P.

Apalachee Center, Forensic and Civil Residential Treatment Services

*Forensic Residential Services in four locations in Leon and Gadsden Counties serve individuals who have been conditionally realsed into the community under Florida Statute 916 *Residents formerly in state facilities receiving psychiatric and/or psychological treatment under court orders are assisted to reintegrate into the community; offers Psychiatric Services, psychological evaluations, nursing services, case management, group and individual therapy; liaison with the courts; designed to help individuals learn skills to successfully re-integrate into the community, or a less restrictive environment then state treatment facilities; learn skills to help residents be successful in their living environment and to successfully complete any judicial requirements *To be eligible residents must be mentally ill conditionally relased (NGI or ITP status) who are 18 or older, ambulatory or able to self transfer and free of major medical contitions requiring ongoing nursing care, able to be an active particiapant in treatment, able to manage ADLs, free of any charge involving a sexual office, and non-violent charges only for jail-diversion programs *Referrals acceepted from Judicial Circuit Courts, state forensic Mental Health treatment facilities, and state civil mental health treatment facilities *Transition Home: unlocked facility in Leon serving males and females for short-term residential treatment for persons needing step-down program treatment, providing psychiatric treatment, psychosocial rehabilitation, nursing and case management *Satellite apartments (8 shared by 2 people) in Tallahassee: residences for clients many of whom attend school or work, supportive environment to enhance independent living skills particularly in the area of housekeeping, medication management, budgeting, cooking, community access and recovery; mental health services provide by Leon County Outpatient Services

Apalachee Center, Homeless Project

*Provides outreach, mental health case management and psychiatric/medication management and supportive services for homeless persons with severe and persistent mental illness who are uninsured with a goal to provide clinical assessment, supportive services, and stabilize functioning *Outreach at the Kearney Center and street outreach to enroll clients where they are screened and offered intake if needed by therapists and social workers from various agencies involved in Big Bend Homeless Coalition *SSI/SSDI Outreach, access and recovery (SOAR) Services to assist eligible adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder with applications and access.

*Serves persons age 18 and older who have a primary mental health diagnosis and meet clinical criteria providing mental health evaluations, medication management, case management, outpatient individual and/or family mental health therapy, psychosocial rehabilitation services (aka day treatment which includes social rehabilitation and basic living skills training), co-occurring disorder treatment for persons with a mental health diagnosis along with a substance abuse disorder, and primary medical care services *Same Day Access Program for new outpatient patients: walk-in or join a video call during same-day access hours for an intake session to begin services; M, W, F 9AM-11:20AM and Tu and Th from 12:30PM-3PM *Provides Medication Assisted Treatment (MAT) to treat opioid and alcohol use disorders
