*Comprehensive outpatient treatment program for Leon County residents 18 to 40 years-old assisting persons following their first psychotic episode or in their first year of diagnosis and their families to successfully find their way to psychological well-being, and to access the services they need in the mental health system
* Team-based approach including a Program Director, an APRN prescriber, an Individual Resiliency Trainer (IRT), Family Education (FE) Clinician, a Case Manager, a Supported Employment and Education (SEE) Specialist, and a Peer Support Specialist
*Provides direct coordination of all medical care, help managing symptoms of illness, immediate crisis response as needed, prescribes and manages medications offers therapy, and as the patient is ready can help obtaining financial entitlements and housing; assistance with housing tasks so a person can live in regular housing alone or with a roommate; learning how to socialize; treatment for clients with co-existing substance abuse (dual diagnosis); employment service and job placement; assistance with legal issues; support, education, and skill-teaching for family members; offsite (in-home) and online (telemental health) meetings available depending on patient's circumstances
