Description:
*Serves persons age 18 and older who have a primary mental health diagnosis and meet clinical criteria providing mental health evaluations, medication management, case management, outpatient individual and/or family mental health therapy, psychosocial rehabilitation services (aka day treatment which includes social rehabilitation and basic living skills training), co-occurring disorder treatment for persons with a mental health diagnosis along with a substance abuse disorder, and primary medical care services
*Same Day Access Program for new outpatient patients: walk-in or join a video call during same-day access hours for an intake session to begin services; M, W, F 9AM-11:30AM and Tu and Th from 12:00PM-3PM
*Provides Medication Assisted Treatment (MAT) to treat opioid and alcohol use disorders
Intake Process:
Call Evaluations and Admissions line; Same-day access available
Program Fees:
Dependent on services needed by patient, accepts most insurance, Medicare, Medicaid, and offers an income based sliding scale
