*Memory screening
*Comprehensive diagnostic evaluation for persons experiencing memory problems
*Neurological evaluation, blood work and other testing (CT, MRI, EEG), medication review, neuro-psychological testing, psychosocial assessment, summary evaluation conference
*Suggested plan of care, referral to community resources
*Support group and counseling; support group meets second and fourth Mondays of the month 11AM-12PM at Leroy Collins Leon County Main Public Library; call for more information
Intake Process:
May request referral from primary care physician or complete self referral
Program Fees:
Some insurance policies may require a physician!s referral to cover the cost of the evaluation; staff will verify your insurance coverage prior to your appointment; no fee for support group or counseling
