*Applications must be filled out on website; agency strongly prefers that any questions be emailed to info@new-eyes.org
*Provides new prescription eyeglasses (basic single or lined bifocals) for children and adults in the U.S who are low income and in need
*Applicants must have an eyeglasses prescription dated within the last 24 months, including their pupillary distance (PD) measurement before applying
*It is optimal for social service agencies/health advocates to apply on a client!€™s behalf; but individuals who do not have access to an agency may apply directly but must provide proof of eligibility
*For students: School nurse may submit letter using school's official letterhead; students eligible for free or reduced lunch are eligible
*Vouchers are good for 3 months from date of receipt and are accepted at on-line provider only
*No longer accepts used eye glasses for donation
