8am-5pm, M-F, Office hours; Therapists work days, evenings and weekends
Required Documents
Any available medical reports, insurance card, proof of insurance approval for ABA therapy
Eligibility
If using insurance, the child must meet diagnosis criteria and have a prescription for ABA therapy
Fees
Medicaid, Medicare, other insurance, sliding scale fee if no insurance
