*Practice is accessible, but cannot be reached by public transportation
*Fees $80 an hour, offers a sliding scale from $60-80 for self-pay; accepts United Healthcare, Tricare, Staywell, Optum, New Direction, Medicaid HMO, Medicaid, Humana, Florida Blue (BCBS), Employee Assistance Program, CIGNA, Capital Health Plan
*Treats Infant/Child, Adolescents, Teens, Young Adults, Adult, Geriatric as family, couple, group, individual in person, by phone and by video (telemental health); specialty treatment areas include general anxiety and depression, marriage/relationship, PTSD, children and adolescents, and eating disorders
*Practice cannot prescribe or manage patient's medications
