Apalachee Center, Leon County Outpatient Services
*Serves persons age 18 and older who have a primary mental health diagnosis and meet clinical criteria providing mental health evaluations, medication management, case management, outpatient individual and/or family mental health therapy, psychosocial rehabilitation services (aka day treatment which includes social rehabilitation and basic living skills training), co-occurring disorder treatment for persons with a mental health diagnosis along with a substance abuse disorder, and primary medical care services
*Program does not accept new clients without Medicaid, Medicare, or other insurance, unless they meet clinical eligibility criteria or have first been admitted to one of their inpatient units (PATH, Detox, or Eastside Psychiatric Hospital); accepts most insurance, offers a sliding scale based on incomoe
*New clients must be referred from an outside source such as a doctor, clinic, DCF, DOH, or mental health agency and have a primary mental health diagnosis, unless they have first been in PATH, Detox or Eastside Psychiatric Hospital
