*Facility to provide follow-up care to certain patients only after they get services from Tallahassee Memorial Hospital or any TMH health facility (including the emergency room), either as an inpatient or outpatient
*Includes patients without a primary care provider, without health insurance, who are not able to obtain a follow-up appointment with a physician within seven days of discharge, who have been hospitalized three or more times over the last 12 months
*Services may include a physical exam, help with filling prescriptions, arranging physical therapy and other types of rehabilitation services, connecting patients with other agencies as needed to help them continue their recovery
