*Intensive, family centered case management services for youth 0-17 with a mental health diagnosis
*Helps families navigate the often confusing and complex systems of mental health care, child welfare, juvenile justice and special education
*Youth are referred directly to providers in the community according to their needs and problem behaviors
*Once services are in place, a consultant monitors them to make sure the services are addressing the needs of the youth
*Interested persons who call should ask specifically for care coordination services
*Eligibility requirements:
*Must be 0-17 years of age and have a mental health diagnosis
*Must be navigating at least two systems (i.e. juvenile justice, medical or mental health, child welfare, special education)
*Must have a confirmed need for services, such as failing school, behavioral issues in the home, recent Backer Act or inpatient mental health stays; referral needs to come directly from a professional working with the family, i.e. school personnel, therapist, doctor, etc. who can express the child's needs
